With the support of the
Federal Office of Rural Health Policy (FORHP),
Rural Health Innovations (RHI), a wholly owned subsidiary of the
National Rural Health Resource Center, is excited to announce that Coteau des Prairies Health Care System of
Sisseton, South Dakota has been chosen to be supported through the
Small Rural Hospital Transition (SRHT) Project. The SRHT Project is funded by the Health Resources and
Services Administration’s (HRSA) FORHP through a contract to RHI.
The purpose of the SRHT Project is to provide on-site technical assistance
(TA) to support small rural hospitals in bridging the gaps between the
current volume-based health care system and the newly emerging value-based
system of health care delivery and payment. Participating hospitals must
reside in persistent poverty areas of the U.S.
Nine hospitals were selected based on program eligibility requirements,
organizational readiness, leadership and commitment to implementing consultant
best practice recommendations that improve performance, as well as strategies
that assist providers in transitioning to a value-based system.
