Transitional Care for Patients
The Swing Bed program at Coteau Health Care System in Sisseton, South Dakota
is a continuing care program for patients who no longer require acute
care, but who are not yet ready to go home. This transitional program
ensures that patients have access to the excellent care they need between
hospitalization and full recovery. Our expert team is here to provide
the support and care you need every step of the way on your journey to wellness.
Services Provided in the Swing Bed Program
The Swing Bed program eases the transition period by allowing the patient
to receive additional care in a hospital setting.
We take a team approach that may involve departments such as:
- Physical therapy
- Occupational therapy
- Pharmacy / medication management services
- Dietary / nutrition services
Our goal is to provide ongoing care and help the patient transition safely
and smoothly from hospital to home. Our care is designed to sustain patients’
well-being throughout each phase of their care. Our licensed social worker
works with team members to assist patients and families with details of
their stay, provide ongoing contact and support, and assist with coordinating
discharge.
